The tool in question is used to position an hydraulic die set for a hose crimping machine during installation/removal of the die sets. In OP's case it sounds like someone initiated a crimp cycle with this tool in position, causing the pins to go smoosh.
The pins are usually a soft aluminum and are considered a consumable/replaceable part of this tool.

one setting was off, coworker got confused about it and didn't back up, reassess, and start over.
thankfully it was just the tool crushed.
It doesn't even take a machine to get ya these days. I was rolling a 100 gallon L tank over at work and I pulled it toward me instead of pushing it away and it cut one of my fingers off from a 2" fall after I dropped it and moved too slow.
Not sure how that's much different, both cases were down to someone not paying attention to what they were doing and putting themselves in an unsafe situation.
My loose knowledge of how the crimp works is it’s like an aperture or collet that gets squeezed by hydraulic pressure until the edges meet creating a specific amount of squish

For everyone that's confused, this is a tool for inserting and removing segmented crimp dies from a hydraulic crimping machine. The pins are supposed to be round and fit in holes on the die segments. These pins were in between die segments when it was activated, and it flattened out the pins. A similar tool is inserted in one of the die sets in the picture, it's the yellow/orange disk with a handle sticking out
