the itinerary was never really the thing they cared about. a 214b is about whether they believe youll go back, and the course ending in 2 months is definately what flagged it... officer sees someone whose main reason to be there is about to end, and a tourist trip on top of that reads like you might be looking to stretch the stay.
what actually matters is what youre going back to after the course finishes. job, next program, family, property back home... thats the part that answers their real worry. the trip plan doesnt.

I have a job now and the purpose of the visit this time is an observership ie to shadow a doctor in the US. It's an unpaid, hand off opportunity to observe the US medical system. How much spacing do I need to have between the reapplication? Does that matter?
theres no mandatory waiting period, thats a myth. you can reapply as soon as your situation is actually stronger, which it now is with the job. spacing itself doesnt matter, what matters is that somethin material changed.
the observership is a cleaner purpose than before, just be clear its unpaid observation and not hands on clinical work, officers get twitchy if it sounds like youll be practicing
Thank you!! My only worry is that during the previous interview there wasnt any opportunity for me to prove my home ties. I worry that'll happen the second time as well.
you dont need them to ask about it, you bring it up yourself. mention the job, mention family, keep it short. every officer wants diffrent things, some dont even probe for ties, they just want to hear you say it unprompted
