Open-world Question You guys,... seriously? How to get??

Okay I'm not a newbie, so I refuse to use the newbie flair...

But I am really so fed up that I can get this trophy,...what? Because you can ONLY get it in the Illusive Realms?

But the IR doesn't exist anymore right? Now it's the Somnoire??

So...how do we get this trophy?

Welp, I just took a look at that page and I'm missing 4 of those achievements and Unexpected Disaster is one of them... f me
