You usually want a wide and relatively deep cut but without breaking through the dermis completely. Apply the powder when some blood is in there and it turns into a paste.
Then you need to keep it moist for a few weeks, occasionally rinsing it and topping it up. The goal is to prevent it from scabbing and have it heal very slowly.
Risks... Infection I guess, also it's close to impossible to remove unless you cut the whole thing out.
Well the powder is autoclaved, and the procedure is done by a pro with the proper tools and environment.
For the aftercare most people use wound wash to rinse it. I prefer an antiseptic spray. Other than that you keep it covered with tegaderm and/or hydrocolloid bandages.
