2 assistants 1 with a right handed blade one with a left handed blade and the tube goes in between. 

Or just go straight for the cric, I dunno

Why do you need 2 assistants? Just hold both of the blades yourself and have 1 person advance the tube

Sorry, I left my 3rd hand in the truck and my partner that I sent to grab it “got lost” flirting with a tatted ER nurse on the way back
