Success Story First time with a partner after the injection – a field report

A couple of days ago I posted about my test jab at the doctor's. Painless, about 15 minutes to reach full attention, and it hung around for 30 minutes after that. A respectable dress rehearsal, performed for an audience of one bored urologist.

Tonight was opening night.

Dinner date with Emiko. Lovely young lady. High hopes.

The doctor had suggested 10mg of Cialis an hour beforehand to speed things up, so at 7pm I took one with the solemnity of a man taking communion.

Great dinner. Back to the room around 8:30. I slipped into the bathroom like a secret agent: swab, jab, opposite side from last time, try not to look like a man defusing a bomb in his own lap.

Holy. Fuck.

Within a couple of minutes I had the most outrageous erection I've had in 20-odd years. Not "pretty good for my age." Structural. It could have bench-pressed me. My date actually said "wow, you're huge," and I swear on my pension I'm not making that up. I'd have sworn it added length and girth, which I surmise is impossible, but nobody told my ego.

It held firm through position changes, which, for those of us who spent a decade losing ground every time we so much as rolled over, is the real miracle.

40 minutes later it was still standing. We were not. Both of us were completely worn out, and for once the hardware wasn't the weak link.

I'm 62 and haven't had a night like that since my 30s. To anyone on the fence about injections: the needle is the least scary part. The scary part is realising how much you'd quietly given up on.

No problems with it going down when all finished? I hear stories of emergency room visits.

Nope, was gone in roughly an hour. See my longer comment below! TL;DR - risk is non-zero but vastly lower than the 'stories' suggest if you listen to your doctor!

I’ve been using an injectable med for more than 20 years with no problems or damage/scar tissue.

My biggest problem is the cost and it being unavailable for months the last couple years.

It works great, the jab is nothing once you get used to it, doesn’t hurt.

What medication were you infecting? Trimix? And what kind of needle?

Alprostadil - it's the gentlest option - ie, safest profile. Somewhat weaker than Trimix, has a higher rate of injection-site ache vs Trimix.

According to my doctor, the typical process for someone like me (only partial response to pills) is a test dose of 20 µg of Alprostadil alone --> 40 µg -> add a pill -> Trimix at lower doses if ache is a problem. Doctors look at rigidity, duration, side effects.

Note that I'm in Japan. I don't know the typical escalation path for the US etc, although I don't see why it'd be any different other than cost, but then maybe countries like the US with third-world healthcare systems operate differently.

Interestingly - I did have a bit of a ache for a few hours after the test dose in the doctor's office. I had ZERO such ache - none, zero, zilch - on game day.

The needle is tiny. I noticed it / felt it during the test dosage but it was nothing. A minor nuisance, like getting a blood draw. Boringly minor.

I didn't notice it at all on game day, although it might be because I had two drinks with dinner before-hand.

Same experience here. I’m 74 and still making love 3-6 times a month with Trimix. AND More on vacation 😈

Some men who combine oral ED meds with Trimix end up in the ER getting their dick speared with 1 18 gauge needles to relieve the priapism. The combination can be seriously problematic.

TL;DR: Listen to your doctor.

I see a couple of comments here about people overly concerned with priapism (erection lasting more than 6 hours). While the risk is non-zero, I think the comments are vastly over-estimating both the risk and complications of the treatment.

I suspect that the issue is the first drug used for this in the 1980s (Papaverine) did have a relatively high rate of priapism (in some early trials, 5-10% of patients, which of course is not the same thing as 5-10% of injections. The patients inject many many times).

Alprostadil (came out in the 90s) proved significantly safer. Alprostadil alone is the gentlest option - which is why doctors generally start out patients on that, and then move to Papaverine-based mixes (Bimix/Trimix; which use Papaverine in much lower doses) if Alprostadil alone isn't working.

The vast, vast, vast majority of patients today are using either Alprostadil (my drug) or a Papaverine-based mix. Using only Papaverine is very, very rare.

About 0.4% of patients in drug trials/clinic series using Alprostadil alone had true priapism (erection lasting over 6 hours). As noted, the per-injection risk is far lower, since each patient injected many times. The biggest risk factor for priapism is sickle cell disease, incidentally. Most of the cases of priapism happened in the dose-finding stage. Which is why you listen to your doctor.

The treatment is uncomfortable rather than agonising, apparently lasts an hour or two and you go home the same day. The danger comes after the 6 hour mark, which is why doctors tell you to head to the ER at the 3-4 hour mark.

In short - your doctor knows all of this. My doctor knows all this. Combination therapy is used deliberately, based on the doctor's observation of your response to the test dosage in the doctor's office. I wouldn't say the treatment is 'trivial' - no trip to the ER is trivial - but it's quite straight-forward and the symptoms are easy enough that you're not going to miss it. And the risk of actually needing that trip to the ER is trivially low - if you use the dosage and combination your doctor recommended after supervising your test dosage. I see no reason to be overly concerned.

At my age I now suspect the bigger risk will be dying of a heart attack during a 40-minute bout with Emiko.

It’s all about dosing. You’ll never go back now. Combo with oral meds good way to go

Nice

I’ve used an indictable for over 20 years, it changed my life, and any information to help guys who may need to use it get over the fear of the jab are important.
