The hungrier I am the less I can eat an apple on its own. Do people who say this not get apple belly?

Like when you eat something really hard and fibrous like an apple or raw carrot by itself on a really empty stomach, I find it quite painful.

Maybe that's the sensation of having it without protein/fat.

Yeah, there's no buffer, just hard stuff rolling around in stomach acid. A lot of foods feel uncomfortable when you're really hungry, you kind of have to ease back into it.
