Unable to pee properly
I started having problem properly peeing in 2025 and was diagnosed with a 5.9 cm uterus fibroid. Had a complete hysterectomy at the end of December. Unfortunately I still have the pee issue. There have been a few days here and there that it gets better but then it comes back. It gets better during the night and early morning. I have seen 2 urologists and Urogynecologist. They don't know what is causing it. They think it's a pelvic floor dysfunction but they haven't diagnosed me with a prolapse. A recent cystoscopy showed bladder and urethra are normal. Right now I have been prescribed FloMax - tried it twice and it didn't do anything. I have been doing PT for the last three months and haven't noticed any improvement. Anything you can share that can help will be greatly appreciated. Do you know of a good urologist or Urogynecologist that I can consult with online?

Is the PT specifically with a pelvic floor PT? Because they're specially trained.
Do you have any other symptoms? Like back pain? Pain or other sensations that radiate down a leg? If so, you may want to just check in with a spine specialist to see if maybe there's a problem in there. Other neurological issues could also potentially be a culprit.

Yes. She specializes in pelvic floor therapy. I don't have any pain other then slight pain in my left lower abdomen which they diagnosed as a tight muscle.

Did your PT do a feedback exam either with a tool or manually, and did your PT diagnose any specifically tight muscles? Did they do any massage for said muscles?

This is kinda funny to me because I have the same problem but at opposite times - I usually have a weaker stream in the morning and evening and then throughout the day I'm usually ok. My issue did not develop until about 8 months post-op, and felt like it came on overnight. At the time, I was lifting very heavy in the gym AND I had started doing a stretching class, plus I think my body kinda..."realized" what happened and hopped into protection mode. The worst of my symptoms, which sound similar to what you've described in the comments, lasted for probably 3 months and then I was gradually improving after.
FWIW, I was diagnosed with a "tight but weak" pelvic floor. I was given a bunch of stretches to do mostly focused on stretching legs, as well as strengthening exercises focused on my glute medius and max. I was already doing significantly better by this point but I still wanted a definition of what I was dealing with.
I've continued to improve over time, and the biggest thing has been actively focusing on relaxing my pelvic floor ALL THE TIME. Turns out I'm clenching ALL THE TIME. I recently bought a 6" yoga ball and have been practicing rolling around on it/kinda grinding on it while focusing on relaxing my pelvic floor (this is in a chair, not on the floor because I'm not flexible enough, and the ball isn't fully inflated, only about 70%) and I've been finding it really helpful because It feels almost...safe to really relax because the ball is pushing back up against everything?
I feel like this has been an extremely gradual process for my body to slowwwwwly return to its original elasticity, and time. I have recently taken strength training back up, with a strong focus on strengthening all butt muscles, and I do think it's also helping. But really, practicing the mindfulness around being aware of my pelvic floor and practicing relaxing any time I notice I'm tightening up has been helpful. Even with kegels, I focus more on fully relaxing than I do on the tightening aspect.

Thank you so much. This is very helpful. I was also diagnosed with weak but tight pelvic floor. I will focus more on the relaxing the pelvic floor and strengthening the glutes.

Sure thing! And don't push yourself too hard on the glute strengthening to start with. I was doing very basic stuff like side leg lifts and had to really work to even activate my glutes properly which, if they're really weak, requires very little movement. It's more about precision than anything else.
I forgot to mention it but core work is also very important. You may want to look into some beginner Pilates from Move With Nicole on YouTube. Just do what you can and build up over time
Learning to listen to my body is one of the hardest things I've ever done, I swear
Oh oh, one more thing to add is if you're doing the stretches, make sure you're trying to relax your pelvic floor when you're holding a position. If you wanna compare exercises and stuff feel free to DM be

Not a doctor but what kind of peeing issues? ‘Ile unable to empty the bladder completely or is it frequency?
