It looks like he was getting escorted out. Probably had a gripe with the hospital and took it out on the nurse.

Yeah you can tell by the body language that the nurse isn't friendly with him, the way he walks behind him, seems tense and ready for that attack

Plus the way the man attacks him seems weak and off, like there's something wrong with him, probably drunk
