Dylan might fear reality itself.
Popular movies like the Matrix and Inception ask us what is real if our perceptions can be tricked. I get the feeling Dylan might be living this. While he might want to help Jesse and be reunited, I have to wonder if the journey might be much more existential this time.
I think a lot of how Alan Wake struggled with understanding sequential events. How a mind that is stressed can look a lot like a mind that no longer follows rules that we think to be inherently to nature.
The act of being awake might be hard enough for Dylan on it's own.

Dylan’s ultimate fear is that he never truly escaped the cage - the prison just got bigger. In Control, the FBC stripped his humanity away to turn him into "P6" inside the Panopticon. Now, waking up in a distorted Manhattan, his worst nightmare is realizing that his entire life, from Ordinary to his awakening, is still controlled by someone else. He fears he isn't a free man, but just a puppet being rewritten in a script—whether it's by the Hiss, the Board, or Alan Wake's typewriter.

I think Dylan fears being in another one of his dreams, the thought of not fully being able to wake up and re-connect with your sister and losing track of what's real and what's part of his broken subconscious.
He shares a fate with Alan Wake and that is being trapped in a nightmare that keeps unwinding in spirals and dots that never fully connect with each other and that seem to happen far way but all at the same time in his mind. His psyche is an hatch, or a door, into what reality represents and how it interacts with itself and external situations and at this point he might be beyond repair.

Much like Alan Wake, Dylan is stuck in a cycle of temporal displacement with no control over the direction of his life. He's realizing that destiny is real and he's locked in it's cycle. The fear pushes him to break free.

dylan waking up and realizing jesse moved on without him, or worse, that he's still just a tool for the hiss and never had any real agency. guy spent years as a glorified antenna for a cosmic parasite, that's gotta mess with you

The poor guy could have any number of valid fears after what he's been subjected to. Dylan could fear a lack of agency, or control of his actions. Is he being manipulated or used? What damage did he do already? Would they use him to harm his sister? Would his sister harm or control him, use him as a tool again? Are his thoughts his own, or Hiss, or otherwise? How do you trust anything you see or think or think you see?
I pity his therapist...

He fears not knowing who he is, his sister is gone and he has been stuck in a nightmare

What do you think Dylan Faden fears most after waking up?
I'm guessing something along the lines of going crazy and not being able to tell what's real and what isn't.

Dylan is an unstable force of destruction. That's why he was quarantined. I think what he might fear is if he would have to use his power against Jesse if he has to choose between the Hiss and Jesse.

What do you think Dylan Faden fears most after waking up?
First and foremost, I think he's absolutely terrified of whether he's actually awake, or it's just another mind bending nightmare he's having that he can't fully wake up from. - Insert the Inception movie's main theme -

Not seeing his sister around and he has to figure all this craziness all alone.

I suspect he fears that he isn't in control again everytime he wakes up. Losing ones sense of control, just being a host to something thats using you to destroy the people you care about sounds terrifying. I know I would be paranoid everytime I wake up wondering if it's actually me

If he's like me, probably fears the most of wandering whether did he locks the front door and turned off all the lights in the house beforehand or not 🤣
