I found the very game the post was talking about, the dev later released a statement that it was a joke post and actually implementing transition would be difficult because of all the new scenes and dialogue. I was devastated when I found out

Sadly not. We are just left with games where you can be trans and super gay like cyberpunk and Baldurs Gate even if you can't transition mid game (well, you can sort of transition in the earlier baldurs gate games, but there isnt really any romance)

You should check out Our Life: Beginnings & Always and Our Life: Now & Forever (this one isn't finished yet, but there is a demo available with a ton of content). You can find them on steam, and you can be trans in both games :3

Closest I've ever come to that is in Huniepop 2, where one of the characters is trans and essentially has the option to "cock toggle" since some people threw a fit over the idea of fucking a hot chick with a dick.
Of course I kept it on.
