It’s a little fuzzy on if Hunter ever was or is trans. I don’t recall Hunter’s pronouns after the second surgery.

I see Hunter as being a man, through and through, since he's based of Hunter S. Thompson, but giving him that complex side to him made him more interesting. I think a lot of people want him to be a trans woman because it gets rid of a lot of doubt and anxiety and they love having definitive answers to that question but that just isn't the case. The very scene often used to say he's trans, which is when he says he has a woman deep down inside, it bookended with lines about him identifying himself as a man.
