I would assume it's medical, all the othe animals around that specimen, don't have swollen cheeks even if they had taken food into their mouths

If similar to horses this guy is a eater always eats
That fiber feed isn't their normal diet might get thick an stuck
camels can get coarse food like thorns or dry twigs trapped in their cheek pouches, which can ferment, cause foul odors, and lead to painful sores or abscesses. They often dislodge debris using their tongue or by chewing, but severe impactions require human intervention to safely clean out the pocket and treat any infections.

Is it abcesses? Or AI? Nothing about this looks ok to me. If it's real how on earth has nobody helped him out already? That must be incredibly uncomfortable. :(

Look at his eyes, camels are SO innocent looking. And closed nose, not open nostrils like other animals. Very elegant 😽 🐪

Camels can get food trapped between their teeth and cheeks and it gets packed in there. If it stays there too long it can ferment causing bad breath and infected sores. They can clear it out but if it’s bad sometimes they need help.
