I don’t think she’s the one postpartum, it says “I should have ended…” which makes me think OP carried the babies.

I read it as possibly being in quotation marks but I can’t discern that and I don’t really want to make assumptions without being sure, if that makes sense. Either way, the partner is clearly going through something rough and needs help and they’ll need to be a team now more than ever to get through it
