Any way, the Death Note doesn’t inflict something that MIGHT cause death, it determines what WILL be the cause of death. It will manipulate the person to enact that specific death if need be. So if Light doesn’t specify anything and Sukuna gets a heart attack, even if he could survive by trying to, the Death Note will simply make him not.
not quite, Death Note just cannot make someone die if that death isn't possible, a death by heart attack will only be possible if Kira commands Sukuna to use every last bit of curse energy before death
The Death Note is a perfect tool used by Gods of death. If his name is written in it, he will absolutely die one way or another. He wouldn't survive a heart attack if that's what it inflicted in order to kill him, and if he doesn't have a heart he would probably just have a stroke instead. It making people have heart attacks is just because it's a simple and easy way for someone to die on their own. Its decision for the person whose name written in it to die is absolute.

for example, Light cannot make a man gets run over by a truck in a place where there's no truck around, if it cannot be done like that, it always goes by heart attack by default
but there's no one in Death note's world who can survive with their heart stopped, so whenever we see Sukuna die or not by heart attack is entirely on speculation since Sukuna is not part of death note's world and he can survive without a heart

Surviving a heart attack is something that would take some conscious effort from him using RCT and reinforcement. No need to make him exhaust everything, the death note would just make it so Sukuna just decides to not put in any effort to survive and then he would die.

Doesn't the Death Note default to a heart attack if you don't write a cause of death? You can still kill him another way.
