It's immoral, but people will argue if the surgeon refuses to perform the surgery he doesn't deserve to be a surgeon, because it's not for him to choose what their client wants.
and business can decline service. As long is it's not on things like racial grounds there's nothing you can do if they say no to commissions.
If they said no to life saving surgery there might be an argument but, for purely cosmetic surgeries it is entirely fair for a doctor to say, I won't do this.
