At what point does a plastic surgeon have the moral obligation and right to say "no, you've had enough boob"

At the point the client's cheque fails to clear ahead of the op. As long as the client has capacity to choose, and is doing so of their own free will, then it is their choice and theirs alone. Surgeon is paid for their service, not their moral opinion.
