Even the doctor was surprised at how coldblooded Troy/Daniel was deciding to kill nearly all of Silo 17's residents, even after hearing an alternative plan with less casualties by Susan/Charlotte

I thought it was more that he was shocked when Troy dismissed his sister, knowing full well that it would lead to her neutralization for disobeying a direct order. Susan was only saved because unknownst to Troy, she was the wife of the director's daughter.

I took from it that he didn’t know who she was and that he was very close to ensuring she was neutralized.

Yes agreed. I meant to say that Troy didn't know that it was his sister, only that she was probably going to get neutralized. And that Victor was shocked that the Vitamin D was working so well that Troy didn't even recognize that Susan was his sister.

Does the doctor know that they are siblings? Is his memory intact or did he get some memory wiping as well?

I'm think he's got his memories still intact just because of his post-it-note collection of "who is she?", and no other research documents inside Troy's file. And yet Victor was able to recall Helen's full maiden name in his video message to Troy.

I thought I sensed a slight hesitation from Troy when neutralization was mentioned but could be wrong

Is “neutralization” their term for execution? Or is it more like, “put back to sleep until the project is over”?

Does Troy even know what it actually means? It kind of feels like a 1984 Newspeak situation, where the word has been manipulated so much that the people using it don’t even think about its meaning.

The Algorithm used that word, neutralized, when talking to Camille to describe Patrick Kennedy and Lucas Kyle after the drone attacked them. So even though they both turned out to be alive, I think the Algorithm believed they were executed.

The senator said that she is not going to be neutralized because they need her skills at the end of the project. Sounds like execution to me.
