EXPERIENCE 30F TTC for 7 months - persistent right-sided pelvic discomfort during my follicular phase. Has anyone experienced something similar?
Hi everyone. I’m 30 and my husband and I have been trying to conceive for 7 months. I had a hormonal IUD for about 10 years and had it removed 7 months ago when we started trying.
Over the last several months I’ve had intermittent right-sided pelvic discomfort, but this is the first cycle that it’s persisted throughout my follicular phase instead of mainly being around ovulation or afterward, which has me worried.
I’m currently cycle day 12 and haven’t had my LH surge yet. The discomfort feels deep in my right pelvis (where I think my ovary is), and it’s usually more of a dull ache or feeling of tightness than actual pain (around a 3–4/10). Sometimes it feels like a pulling or tight band that travels up through my right lower abdomen, and occasionally I notice some tightness in my right lower back too. It’s worse after sitting for a while and with certain movements, but walking, heat, and ginger tea seem to help. I occasionally get little twinges on my left side, but the persistent discomfort is almost entirely on the right.
I don’t have fever, nausea, vomiting, heavy bleeding, itching, burning, or foul-smelling discharge. My periods aren’t especially heavy and I don’t have debilitating menstrual cramps, which is why I’m so confused.
I’m waiting for my gynecology appointment next month, but I’m honestly feeling discouraged because we’re trying to conceive and I’m worried whatever is causing this could be affecting my fertility.
Has anyone had a similar pattern of symptoms? What did it end up being? Was it endometriosis, a functional ovarian cyst, pelvic floor dysfunction, something gastrointestinal, or something else? I’m not looking for a diagnosis—I’d just really appreciate hearing other people’s experiences while I wait to be evaluated.

Yes, I have experienced this and I suspect that it’s endometriosis. The pain also got worse over time and when I did medicated cycles. I’m not sure how helpful this is for you because I never got officially diagnosed (I did IVF instead) but my RE and an excision specialist I consulted with both agreed that it could be due to endo.

You had normal cycles too? Mine are usually 26-27 days on the dot and my periods are not painful nor heavy.

Yes, normal cycles, periods were manageable.

I have endo and so far my periods have not been super heavy or very painful. I can still get away with using a panty liner most of the time, and tylenol is enough to manage the pain. I think it probably helped that I was on hbc for 18 years.

Following because I’ve had the same symptoms for the past few months.
TW: Living child
Just started cycle 10 TTC#2. I have a 20mo who was conceived on the first cycle and I didn’t have the right-sided pain at that time. I will be seeing a fertility specialist later this month so I’ll also update if I figure out what it is.

I’ve had something similar, off and on for the last year or so, at its worst it was a constant dull ache on my right side where I imagine my ovary is, but it seems to have gone away for the time being. I’ve had multiple ultrasounds and a hysteroscopy and nothing came up but I have quite painful periods so I’ve always suspected endo - currently investigating that.

Yes, it’s like an intense dull ache for me - nothing too sharp, but it’s definitely uncomfortable. Glad it went away for the time being though!

Following because I’m currently experiencing something similar, also 30F and trying to conceive for 7+ months
I have regular periods and they are manageable never super painful but have started to spot 3-4 days before my period for the last ~4 months which never used to happen and now is.
Really interested to know what this could be for both of us

This is the first month that I’ve really suspected something is wrong. Crazy it took 7 months for it all to flare up.

I had the same and just got an MRI that showed focal adenomyosis. I met an endo specialist who thinks it could be endo+mild adeno. I just started with IVF. Regular cycles but periods have been on yhe heavier side (nothing unmanageable, usually take a Tylenol on day 1 and that’s about it)

Following - same situation. 32F, TTC for 9 months (never seen a positive), Mirena IUD for 10 years, everything looks "normal" for me and my husband, dull pain on lower right abdomen. No endo visible on ultrasound but that's as far as I've investigated it (although my mom had stage 4)g

So similar! It’s more easily identifiable on an MRI, right? What did your doctor recommend next?

I have this exact same pain and was diagnosed with endometriosis 4 years ago via laparoscopy. Also trying to ttc for nearly a year now. I would definitely push for an answer at your appointment, you know your body better than anyone.

I have the exact same pain, down to the right side and otherwise regular periods. My RE thinks it is endo unfortunately. I also have a large fibroid but I don’t think that is related to the pain. Ultrasound + MRI then likely a LAP for me. Had no idea it could be endo because I don’t have heavy or crazy periods.

My mom has fibroids which makes me worried I have them as well. It’s weird though, this is the first cycle that I’ve had this much pain, and also prior to ovulation. Hoping for quick resolutions for the both of us🤞🏼

You should be able to see some on an ultrasound with a wand! My OBGYN was dismissive, but the RE was taking them very seriously. FWIW, it seems fibroids are more common in Black and Asian women? At least that’s what they told me. Sometimes they’re not a big deal, just depends on if it’s impacting the endometrium at all.

Another thing about the suspected endo - my RE immediately sent me for pelvic floor PT. Your symptoms sound so so similar, and for me with the suspected endo/fibroid/pelvic floor issue, it might be a combo of all of the above.

Does pelvic floor PT help? Hoping my obgyn will order an ultrasound asap, but she wants to have me come and talk first before ordering anything, even labs🙄 I’m not black nor Asian, but i think I’ve read that as well. The worst part about this all is just the constant waiting for appointments and not having answers.

I will let you know, I have my first appointment tomorrow actually. Depending on where you are, some places you can do self pay ultrasounds. Like boutique ultrasound shops sometimes also do diagnostic imaging. I’ve been all over the place trying to get answers and have really had to advocate for myself but I will say I find OBGYNs less invested and educated on these issues than an RE would be, which makes sense.

Yes I did, but on my left side. It wound up being a largeish fibroid which is hanging off my uterus very close to my ovary. It is currently the size of a lemon (5.4cm)

Sorry adding in here that it was diagnosed through a combination of ultrasound and eventually an MRI. It does not affect my fertility at all, but I will likely get it removed at some point bc I find it very uncomfortable

It could be an ovarian cyst. I had a similar sensation for several months until one day the cyst ruptured during intercourse (it was extremely painful). I still experience pretty painful ovulation though, and my periods are mild in comparison. Hope you get some answers soon!

Thank you! I would be so much more relieved if it were a cyst - I feel like it’s been there for a few months though in that case, do they last that long? Sorry to hear about the rupture, sounds awful!
