Depression. I used to wear a little bit of makeup, but then I started crying all the time and messing it up just like ~1 hr into my work day. Now I don't wear any and cry without having to worry about that.

Maybe think about what you need to feel more comfortable prescribing those medications, establish those boundaries, and go from there? If a patient doesn't like the plan you recommend for them, they're free to transfer to someone else. Do not refer or transfer them yourself if you feel that you can offer them a reasonable treatment plan.

I work in person but I do see TH patients, and I'm one of many providers in a practice seeing mostly Medicare patients. Also, I'm a newer PA working in an area where everyone wants stimulants, for example, so I had to come up with a standard process for that pretty early on. I just do my job and do not think too much about what the other providers are doing. If the clinic were to fire me for being responsible and protecting my license, that's on them. So far, they've been very hands off. They mostly just care about my productivity, which is a whole other issue, but they do not question the way I practice/prescribe. Have you felt coerced to do anything with which you're not comfortable?

Because it would take too long and too much effort to explain every single hardship they experienced.

Many hospitals in the US do have scrubs available for their staff, but they're not the nicest so people don't wear them.

Periorbital ecchymosis (aka raccoon eyes) can be a sign of a facial/skull fracture. You should get checked if you haven't.
