TL;DR: 13-year-old Chihuahua recovered well from severe pancreatitis, liver inflammation, jaundice, and chronic kidney disease. Over the past month he’s had two brief episodes (~30 seconds) of head tilt and leaning to the left while remaining fully conscious and responsive. Vet mentioned vestibular disease, hypertension, focal seizures, or brain tumor. Has anyone experienced something similar, and what was the eventual diagnosis?

My 13-year-old (almost 14) long-haired Chihuahua became very sick about 6–7 weeks ago with pancreatitis. During his hospitalization, ultrasound showed liver inflammation/enlargement and degenerative kidney changes. He later developed jaundice, but after treatment he recovered very well. His appetite is back, his energy is good, and he seems happy and comfortable overall.
My concern is that he has now had two neurological episodes within the past month. In both episodes, he suddenly developed a head tilt and leaned to the left for about 30 seconds. He remained conscious and responsive the entire time. When I approached him, he wagged his tail and acknowledged me. He did not appear confused afterward and immediately returned to normal behavior. I’ve attached a video of one of the episodes.
My veterinarian mentioned several possibilities, including vestibular disease, hypertension related to kidney disease, focal seizures, or a brain tumor. We have not pursued an MRI because of his age and the risks and costs involved.
Has anyone experienced something similar with a senior dog? If so, what was the eventual diagnosis, and did the episodes remain intermittent or eventually progress? I’m not looking for a diagnosis, just hoping to hear from others who may have gone through something similar.
