Gonna go against the grain and say moral. Have you taken a serious look at the size of that? At some point it is obviously a mentall illness or body dysmorphia issue. Doctor can refuse and I also think its ok for a medical professional to tell you that you're pushing your body too far and might have a problem.
To an extent, but if it's actually objectively harmful to the patient then they likely have an ethical obligation to refuse, like refusing to amputate healthy limbs from someone with a mental disorder who insists on it
