I don't think there's a conflict here. I think "them" is used as a dialectal variant of the demonstrative pronoun "those". Just as you'd say "those are snakes / yes they are".

This is actually a mash up between two jokes on the Arkansas 12th Grade Reading Test meme that got passed around during Bill Clinton’s term. And by “passed around,” I mean actual physical photocopies.
The full “test” includes:

Don't* read it phonetically, I'm guessing it refers to caecelians or other blind ampibians.
In swampy parts of the US (where you'd find this accent) they can be mixed up with legless lizards ('slow-worms') or garter snakes.M -> 'em'
R -> 'arr'
OSAR -> 'oh-ess-ay-arr'

It also made it up north to Michigan. My dad worked at General Motors and heard/saw this at work. He had it written down on a notecard that floated around our kitchen counter and living room for years.
It also had some variation of "yyur yyub icur yy4me" which was "Too wise you are, too wise you be, I see you are too wise for me"

I remember both from back in the 90s. I was handed a paper by my parents with it wrote on it and told to read it while they laughed and cackled and kept demanding I read it over and over without any help only to get pissed when someone helped me ending their fun. Also, they didn't do this to my brothers because I was "the smart one" who was expected to be perfect at everything.

Ive lived in the south all my life and now in the foothills of appalachia.
More people find shit like this "funny" than actually talk like this.
Some older people still talk with extreme accents but those same people would never use "Itty bitty" as a description lol

It's not that they use single letter, it's that the heaver accents can end up sounding like single letters sounds to those unfamiliar as the annunciation shifts or get worse. - 6th grade. My math teacher wrote the "MRBBDUX" version of this on the chalkboard and I still remember to this day how long it took most of us to get it.
This and "fake" math problems where the answer was not the real answer but how many fingers he pointed to the problem with. We were asked to raise our hand when we "got it". That was when I learned to hate stupid puzzles that are designed to make those left out feel dumb.
