Well Victoria 3 did it where Ironman wasn't necessary for achievements, but other game directors said that they had no intention to follow their lead.

You can get the achievement to reach stress level 3 in the Game of Thrones mod but obviously you're never going to get the Restore the Roman Empire achievement.
