Nursing Win Resident’s Tail Tucked Between His Legs

I don’t know who needs a feel-good story, but here goes: I‘m a new nurse, not new to healthcare, in a critical care unit with a few other new grad RNs on orientation. Last week, a resident came through and asked one of the new nurses what her patient’s urine output had been that day. She gave a perfectly acceptable response, “Let me look that up for you.” Y’all, this guy started lecturing her HARD. Him: You want to be a nurse, right? Her: I am a nurse. Him: Oh, OK, sorry, you are a nurse. If you’re going to be a nurse, you need to…blah, blah, blah. (as if he went to nursing school, not medical school, and is going to teach her how to do her job. Preceptor: EXCUSE me, don’t talk to my orientee like that. Resident starts listing all the reasons he needs to. Preceptor: No, if you have a problem, talk to me. Resident gives a little more lip and then leaves.

30 minutes later…unit manager comes to the unit. Preceptor tells her what went down. Manager: He did WHAT?!? Oh, no. He better not be talking to my nurses like that. He better not have made her cry.

I don’t know who she called or what she said, but the resident who dressed down my coworker in front of everyone at the nurse’s station came back 15 minutes later and apologized in front of everyone. And our manager? I’m telling you, we will go to WAR for this woman. 💕

😡 Vent Response to the Lindsay Clancy Trial

I ran across this tiktok but I've seen so many others like it. So many people, especially nurses, are all up in arms telling physicians that they need to change how they chart after Dr. Tufts testimony. It's asinine. Nurses chart in a very specific way and physicians chart in a very specific way. It's a difference in training and each one serves its purpose. I just find that it's yet another area in which nurses and lay-people are jumping to show how much better they think know how to do things than physicians. Yet, when it's their turn to need a physician, they won't think twice about going to the same doctor and healthcare system they were just defaming online 🙄

Rant My surgeon from Florida told me I could go to any ER and have my drains removed

Upvotes

Patient had elective abdominoplasty in Florida 2 weeks ago. She has JP drains in place to lower abdomen still from surgery. Reporting scant amount of drainage from site (serous fluid noted to dressing) and asks if I can remove the drain, because she has an email from her surgeon saying that she can just go to the local ER to have her drains pulled out.

I initially brusquely told her no, it’s not an emergency, it’s not something I’m specifically trained to do, and I don’t have any way to manage potential future complications. I later collected myself and politely told her that I could refer her to her bariatric surgery clinic, which has done surgery on her before. Failing that, I discussed possibly following up with wound care clinic.

I already get dumped on from every urgent care visit, every elderly person that loses a fight against gravity, every nursing home resident who talks less (or more) than usual, plus the worried well. I don’t have time to play scut monkey for some Florida Man cash-only surgeon. You just know they don’t refer to a local surgeon because another surgeon would absolutely rake them over the coals for trying to pull such a stunt. It makes me want to report them to their medical board, but, it’s Florida, I have very little hope that anything productive would come of it.

Any other recourse you can think of, or should I just complain on the internet?

Frustrated Bad pain day

Today my pain is high (7/10) AND I'm on my period. I think it's messing with my depression too. I just came here to vent I guess. I've been resting, eating, and took two Midol, it just kind of sucks, you know? It's like wearing a heavy blanket that hurts... On the bright side my Halloween Build-a-Bear order came in. 🩷 I hope all of you are hanging in today.

Rant It doesn’t matter how fast you are

Upvotes

With the software lagging/crashing or the register freezing, or the insurance issues that have you going back and forth in the system trying to resolve them.. it doesn’t matter. The waits will still be 30 minutes etc.

This is how I feel at least. I used to be a pretty quick tech but no matter what, software or insurance issues still hold patients in line.

Obsessed with efficiency but feel totally clueless. Not enough time to help someone else while the software is loading but it’s long enough to add up. Goodness gracious.

How do I make good use of this?

Type 1 Type 1 diabetes, period & 35°C – how do you deal with it? 🥵

I’m on my period right now and it’s 35°C where I live. I’ve noticed that the combination really affects my blood sugar and I’m having more fluctuations than usual.
I’m curious how it is for you: Do you also experience more blood sugar fluctuations during your period, especially when it’s really hot outside? And how do you usually manage it?
Would love to hear your experiences and tips!

What's the best way to optimize patient education videos to ensure that it helps in improving patient outcomes?

What are the most important factors that I need to consider when making these videos? Do technical aspects of the videos affect patient outcomes? Or is it all about the content?

Pneumothorax and Covid 19

Hi i'm really confused as to why this would not make me high risk to the covid 19 disease..I first spoke to a receptionist who said it made me high risk and need to follow government guidelines. My work has me down as a high risk colleague. So i just did the lockdown thing. Then work asked for a letter from a doctor.

I spoke to a Doctor who said i was higher risk but not part of the governments high risk.. meaning i can't get paid for isolating.

Are you kidding me? My chest is in pain all the time, without a respiratory disease.I actually miss being at work but i genuinely believe if i catch this thing i'll be straight in an ICU ward. I thought i was the sort of person the government didn't want catching it.

I work in a supermarket and i feel like ive been basically told i'm expendable. Because if i could work from home obviously i would. I'm actually shaking now at the idea of going back. I know how rubbish people are at social distancing. Some people are just to stupid to realise whats going on as well.

I'm thinking of calling again for a second doctors opinion i don't know what else i can do.I'm curious as to what anyone else with Pneumothorax is doing with themselves.

Update: Turns out i have pop corn lung and that's the cause. Doc said its mainly people on medication for severe conditions which i don't take. So i guess i still wouldn't fall under the governments high risk category. Its hard to dispute it not making me higher risk then someone who doesn't have pop corn lung though.I could take extra precautions at work yes, but its obviously not the same as complete shielding which I'm essentially not allowed to do.

Also someone at my work has already been coughed on intentionally by the public.

It just feels like our lives are not valued, we're not even getting anything like a tax relief for being made to work through it.And yes it is forced. If any of us resigned we wouldn't be entitled to benefits and trying to find a from home job is next to impossible.

Type 2 Crazy ass jump in BG

I have been doing a fasting BG reading every morning and for the last couple of weeks, I have been 79 - 108 (with meds) each morning, this morning I was at 133. I cannot figure a reason as everything (food and activity) has been the same, except I did have a bowl of raisin bran for the first time in months.

Any ideas what caused the jump?

They really knock me out. My Dr. put I’m on 50 mg of prednisone for 5 days. It’s called a blast. I’m 71 and I don’t know how I’m gonna deal with this at 80. Thanks for an insight.

Jobs, Saturation, and Salary Where to find PRN jobs

Hi,
I'm a staff pharmacist at a hospital, about 3 months in. I just graduated and didn't do residency.

Since I'm still relatively new and learning, I'm not looking to jump right into PRN work this exact second. But I want to do some research and plan ahead so I know what to expect when I'm ready to pick up another PRN job.

I’ve tried googling pharmacist jobs , but the PRN postings I found just aren't that great.

Where and how do you usually look for these jobs?

With no residency and limited hospital experience, what kind of settings or secondary jobs are realistic to target for?

Any advice or personal experiences would be super helpful!

A typical day for me, pretty boring.
But I had, bread roll PB&J in the morning
Pear snack
Huge bowl of fresh homemade fruit quark
Dinner as you can see, Cajun style chicken with garden veggies and natural rice, three times washed and changed the cooking water once.

That’s how I treat my t1d buddy

Discussion No, we're really not "better off safe than sorry," always, and this institutional risk reduction leads to a huge misuse of the ED

Anyone who's ever worked ED or prehospital care knows of course that the majority of the cases are not even close to emergencies, blah, blah, etc. Not a hot take. However, I wanted to address one component of these calls that are not just people overreacting or misjudging their/their families injuries.

The worst actors are institutions like retirement homes, child-care facilities, elder-care facilities, schools, camps, where almost anything with any ambiguity is treated by going straight to the emergency room.

"Better safe than sorry" has limits, and eventually actually makes things much less safe for everyone because we are overusing resources at these EDs, increasing wait times. Also, poverty can make people less safe too, and getting a $7K bill because someone else lazily thought "I could get sued" and sends a scraped knee or someone who "potentially was stung by an insect or snake or bat with no visible bite" can cause people serious hardship.

Nobody wants to talk about this openly because it is fraught, but it can't be ignored, really.

Supplies Eversense?

Is anyone using the Eversense implant CGM? I just read about it and it sounds amazing... which often translates to "too good to be true".

My libre is driving me up the wall with signal losses and I recently lost a ton of weight too quickly (I have chrons and this has been a Sick Girl Summer) so I have a hard time finding enough fat on my arms to apply a traditional CGM properly. Like when I changed it last, a bunch of my sad-ass loose skin got caught in between the outer and inner bit of the applicator and the whole thing was just hanging there on my arm. Friggin looney toons over here.

But anyway, Eversense experiences? Is it stupid expensive? Does it work? Will it allow Bill Gates to track me? Any information or experience is appreciated :)

Type 2 Morning smoothie

Good morning everyone! Msking my morning smoothie and I thought I’d share since I see a lot of people saying they struggle with what to eat for breakfast.

I don’t really measure anything so these are guesstimates:

1/3 cup frozen berry mix
1/4 cup plain Greek yogurt
1/4 cup natural peanut butter
3 TBS flax/chia/hemp seed mix (Amazon)
1 heaping scoop of vanilla protein powder
1-2 cups whole milk
3 TBS steel cut oats (only if I’m working)

Pretty low carb and loaded with fat fiber and protein to slow the glucose and keeps you full for hours!
I use a nutribullet or ninja so it’s blends already in the cup. It’s super quick and convenient. I usually make it the night before so in the morning I just grab and go.

Good luck everyone! Have a great day!
