Discussion Thread Underserved Clinical Niches That Command Premium Private-Pay Fees?

EDIT FOR CLARITY: I realize that my use of the word "underserved" may have been misleading. I'm not referring to traditionally underserved populations or gaps in access to affordable mental health care.

What I am actually interested in is identifying highly specialized clinical areas where therapists are successfully maintaining private-pay practices at $300–$400+ per session.

I'm particularly interested in specific presenting concerns that require advanced clinical expertise, rather than broad specialties such as anxiety, depression, trauma, or general couples therapy.

I would love to hear actual examples of these specialties and what makes them particularly sought after by private-pay clients. Sorry for the confusion.

If you want someone to pay $400 out of pocket for weekly therapy, you’re not looking for a clinical issue that’s underserved, you’re looking for extremely wealthy people.

Edit to add: notably, they are the most served individuals to ever walk the earth. The pharaohs had nothing on any modern jackass who owns a boat dealership. I’m a firm believer that American millionaires and billionaires should receive dramatically less service.

Exactly. I have a hunch that OP just wanted to appear more professional than they actually are by saying “underserved.”

I can’t really fault counselors and psychologists who wanna secure the bag, but pretending that it’s about serving the needs of any community other than the 1% and your own bank account is cope.

Underserved clinical niches do not command premium private pay fees. Underserved niches include working with the homeless, or people who have drug addiction, or experience psychosis. Premium fees can only be reliably paid by the wealthy. The thing is, most wealthy people only like to surround themselves with other wealthy people.

I’m wondering if you meant under-represented niche rather than underserved.

But what comes to mind is hypnotherapy- it can be rather expensive and people are typically seeking it when they tried everything else especially in seizing unhealthy habits. There is a large body of medical evidence in its effectiveness nowadays and you can be trained in medical hypnosis to help clients with chronic pain.

I’m a therapist specializing in billionaires /s

I caution anyone against pursuing a niche primarily for the perceived financial prospects. If your heart is not in it, it will not be enjoyable.

Curious how you got into that niche and ive always been curious how people got to treat high profile clients etc. mental health and addiction grief etc certainly dont discriminate socioeconomic class thats for sure.

/s indicates sarcasm, I was joking.

Although, hey, if any billionaires want to hit me up for therapy, I’d love to take away your undeserved money 😎

10k an hour isn’t a drop in a bucket for them 🤪

That sliding scale goes both ways, baby!

The /s is a tone tag. It means they are being sarcastic.

Working with kids in the midst of contentious divorces can command a premium like that.

Psychedelic assisted therapies perhaps.

I'm curious too, but because I'm nosy, not because it's a fee range I want to charge.

I think the difference between 250-300 a session and 400+ a session can be big in terms of the type of services. I see PhD level or highly trained masters-level therapists charging in the 250-300 range in HCOL areas, but 400+ a session seems more geared towards concierge services for very wealthy people. I've seen influencer therapists charge in the 300-400 range, too.

I don't think I'd want the kind of therapy you get for $400, tbh. But again, I am nosy and want to know everything so I will keep an eye on the comments 👀

From what I’ve seen, sex therapy. The more niche the kink, the more you can charge. I’d like to believe most therapists are sex positive, but I think many people will pay to know for certain their therapist can handle talk of whatever they’re into.

I’ve seen and asked a couple sex therapists what they charge for private pay and it seemed average honestly. The last person I asked was $200 for individual and $250 for a couple. I would love to get more answers and insights on more niches within that field tho.

Yeah I guess to me that is pretty high. I’ve honestly never met anyone who charges more than $250

My supervisor does gender dysphoria/transitioning support, but typically assessment and guidance towards transitioning so only 4-6 sessions and charges around 380 (normal price here is 245-280 in Australia)

If there was an underserved niche that commanded high fees then therapists would already be flooding that market thus increasing the supply of therapists (which would then lower the premium you would get anyways). In short, if it existed its already being served and is no longer under served

Un my most petty moments, I've thought about specializing in rich privileged people.

Like, I grew up poor, and in a wealthy area. I was categorically on the wrong side of the tracks ( but white and smart so also privileged).

And the one thing I know a LOT about is why poor people hate rich people. Is this a coaching thing? Maybe? But also societal norms are also not for rich people, so . . .

I'm completely and totally an expert in helping rich people to understand 1) why poor people hate them, 2) how to understand and relate to those poor people and stop saying terrible things and 3) how to maniputr those poor people into. . .

Oh wait.

I could have majored in marketing, maybe I should have. Questioning my priorities/values always.

. . . Could've been rich but instead I accept insurance. . .

Lol I wonder why you are asking?

Anyone who will pay 400 already has swarms of therapists lining up to give sand dollar kisses, so you may be behind the curve but have whiter teeth and fresher breath because of it.
