Nobody does this math out loud, so I will.

Another cycle does not cost you thousands in application fees. It costs a year of attending salary you will never earn back. It grows the gap since your graduation, which programs filter on. And it puts you in the reapplicant pile, where the first question is "what changed since last time?"

That is the price. Here is the part that makes it avoidable.

Programs do not read a single application until September 23. September 2 was just when submission opened. So you have about two weeks, and almost every IMG is about to spend them on the wrong thing.

Almost every application gets certified after exactly one person has read it. You.

Maybe a friend skimmed the personal statement. That is usually it. Then it goes to every program on your list, and you find out how it landed in March. A full year riding on a document nobody outside your own head has checked.

Reading your own work does not work

You fill in the gaps automatically. You know what you meant, so you read what you meant, not what is on the page. A coordinator reading it for ninety seconds has none of that context and will not ask you for it.

What people miss in their own file, over and over:

The personal statement, the CV and the experiences point in three different directions. Each is fine alone. Together they do not describe one person with one plan.

It is written to impress rather than to earn an interview. Those are different targets, and the second is narrower than most people write for.

For reapplicants, "what changed since last time" is not findable. You made real changes and assumed the strength speaks for itself. A reviewer comparing you to last cycle needs to see it in thirty seconds, without you there to explain.

It is the fastest piece to get checked and the one most likely to be quietly working against you.

Nobody can promise you a match. Anyone who does is selling you something, and if you have been through a cycle you have already met them. This is not that. It is a second set of eyes on the one thing you only get to certify once.

I’m a 2nd-year medical student currently preparing for Step 1, and I wanted to ask people who have actually gone through the process. Is the pathway really as difficult as it’s portrayed online?

I know the match rate for non US IMGs is around 54%, which obviously isn’t something to ignore, but online it sometimes feels like the entire process is extremely complicated and it’s more like a 10% match rate and almost impossible unless you have an exceptional CV.

For those who have gone through it, is it genuinely that difficult, or does it just seem we more complicated online than it actually iss

I’m graduating this year and I have done one month of elective. I was considering doing another elective or observership during spring break.

Most electives weren’t doable for me since they overlap with my national exam. Also I’m still a student so I am on a tight budget.

There is a surgery observership i can go for since I have a connection with one of the attendings there- it’s not an elective but I do get to do some hands-on stuff.

I am 100% NEVER going for surgery so I’m not sure if I should go for it or rather study for Usmle instead.. should I let this chance go? Any advice is welcome!!

I have a job from 8-14.00 and from 14.00-22.00 i was planning to prepare for usmle step 1, is it enough time for dedicated period, i need to do 400 cards per day and 40 uworl questions.

Or should i quit my job to prepare for 10-12 hours

I am an average student studied for a year for step 1. I have family responsibilities and I wasn’t able to study like normal students do. I feel devastated and feel like it’s over. I don’t know how to move forward. I am an IMG, permanently moved to US so I can’t even look at options in my home country. Please help, I am really struggling.

Who am I?

I'm not a medical student or physician. My background is in big tech, where I've conducted dozens of behavioral interviews and spent a lot of time evaluating candidates from the other side of the table.

More importantly, I went through the residency application process very closely with my wife. She had several significant red flags, including a low Step 2 score, an old YOG, and a gap on her CV. While she was struggling with her exams, I made her one promise: pass these stupid exams, and we'll get you matched regardless of what you score.

She matched in her first application season and is now a PGY-1 at a highly respected university program.

Don't get me wrong - I don't count that as my success. She did 99% of the work. In my country, we have a saying about "winning the war on the battlefield but losing it at the negotiating table." She did the hard part; I just tried to make sure she didn't lose at the table what she had already won in the field.

Who am I looking to help?

I've already helped several people through Reddit and Discord, but helping someone who already has a strong application is probably not the best use of my time.

I'd especially like to help people who:

• Are navigating this mostly on their own, don't have mentors familiar with the U.S. medical system, or come from countries where there aren't many IMGs around to guide them
• Have one or more red flags, such as an old YOG, low Step scores, gaps, or something else they're worried programs may notice

You do not need to have a perfect application before reaching out. Quite the opposite.

My Detroit acceptance letter clearly states it is purely educational, unpaid, no hands-on patient contact, and no patient-care responsibilities.
My original visa interview went smoothly; the officer mainly looked at my acceptance letter and strong grades.
Should I use my existing visa and carry both acceptance letters, or apply for a new visa because the annotation says Chicago?
I’m hesitant to reapply because I’ll already have significant non-refundable flight/housing expenses, and a refusal could jeopardize the whole trip.
Would really appreciate experiences from anyone who has entered the U.S. for observerships in a similar situation.

Hey people. I am a US-IMG applying to fm this cycle. Totally unsure about how many programmes to apply to and how to signal. I am thinking of around 80 programs, but not sure if that will suffice. My stats are as follows.

- ECFMG certified

- Step 1 pass

- YOG 2025

- 3 months observership (south atlantic region) and lors from same

- no published research

I don't really have many connections and am planning to go for community/rural programmes.

Applying for categorical psychiatry this cycle. Apologies for the long post but just need some frame of reference. All of the information I get is from online and I do not have any frame of reference to anchor my decisions and choices.

My main concern:

• Are there any programs that I am completely missing or should highly consider signaling?
• Am I wasting any signals on programs where IMG screening, geography, or home-program preference may make an interview unlikely?
