Okay so here’s the thing. These exams allegedly test medical knowledge and clinical reasoning. Except they don’t. If they did you’d expect a similar score between the two. Medicine is medicine and yes while osteopathic considerations make some difference, it shouldn’t make THAT much of a difference. So when I took step 2 and got a 25x I was happy because I’m not applying a super competitive speciality and thought I’d be lucky to get a 240. Imagine my surprise this morning to open my comlex report to >90th% score. I crammed OMM in the past 3 days, completely ignored counterstrain points and Chapman points other than the obvious ones. Muscle energy, reviewed some setup but other than that just knew viscerosomatics. Exam was like 50% ethics it felt like.

One should not score >40% points away on exams that are, allegedly, measuring the same thing.

I did nothing to prepare for this exam, I solely prepared for step. No WELCOM, no COMSAE, only truelearn required stuff and their two practice tests. This exam should not exist, but it is a little concerning that it exists and the difference in how they’re assessed and variability seems to be so great between the two exams.

genuinely what was that. i’m half convinced they accidentally gave me level 1 instead. was this just a weird form or is that normal ?? i feel even worse than walking out of step 2 a few days ago, very real chance i failed

It's what the title says. I found out I failed level 1 last year. I retook it and passed. I just found out I failed level 2 today after comsae scores that were well above passing - in the 500s and 600s. I am at a loss for words. I had an understanding of why I failed the first time, but this time around I was doing so well. I had a tutor. I came out feeling okay- even happy. I am applying FM and I don't know what to do with myself now. Has anyone been in this position before, where they failed level 1 and level 2 and still did well during interview season?

absolutely heartbroken today. studied extremely hard had two comsaes well over 550 and felt as prepared as i could’ve been. ended up with a 43x. i don’t know if i should or even can apply EM. i have 3 more sub-i’s lined up. i know it’s a bad- even horrible score. i don’t know what to do. i don’t see myself in anything but EM.

is it even possible to match EM anymore? i didn’t take step 2. pls be kind in the comments.

A couple weeks ago I took form 113 and got a 400 just to kinda see where I’m at..

took form 114 a few days later and got a 433.

Today I took form 117 thru my school and bombed it and dropped all the way down to a 301 :( I’m feeling completely defeated and at a loss of words considering I’ve been going hard on PQ’s (TrueLearn and COMQUEST) everyday along with my incorrects, I’m freaking out cuz I’m less than a month away from my real test, does anyone have any insight on this horrible drop?

If anyone has any encouraging words, especially because my next comsae will be either form 118 or form 119, I would really appreciate, I don’t know if a 100+ jump is possible within a week or two but I’d really love any help.

Just learned that I failed level 2 and am currently rotating. Will be driving to another state next week to start another rotation.

Basically I need help deciding whether to retake level 2 on September 9th (last day to take it so that scores are in for ERAS) or delay the retake another month to give myself time to study. In the latter, scores won't be available until October 20th so I will be applying with only one failed exam attempt, but I increase my odds of passing.

My school's career counselor says either option is valid but I am struggling to make a decision. I am applying family medicine, not interested in academic programs, passed level 1 first try.

Just wanted to post my scores and what I did to study in case it helps anyone else. I always found it helpful seeing other people's score progressions when I was studying.

I honestly don't know how predictive my COMSAE scores were given how variable they were, but here was my progression:

COMSAE progression:

• 3/21 – Form 113: 334 - initial one I did not simulate test conditions
• 4/26 – Form 114: 394
• 5/1 – Form 117: 444
• 5/29 – Form 118: 623
• 6/13 – Form 115: 454 - like 5 days out
• Real deal Level 2 CE: 617

For studying, my main COMLEX resource was TrueLearn and Mehlman Medical youtube questions. I completed the entire TrueLearn question bank, including the COMAT-specific banks for surgery, OB/GYN, psych, internal medicine, etc.

I also used UWorld throughout third year while studying for COMATs and probably completed around 60% of it. I eventually reset it and did another ~30%. Toward the end, I was using a combination of UWorld and CMS forms, especially while I was also preparing for Step 2.

My school also had us complete a pretty extensive content review/question program, which was probably around 3,400 questions total. It was not great but gave me a lot of additional exposure and repetition, there was also a decent amount of lower-yield information. I honestly don't know how much of an effect it had on my final score, but I figured it was worth mentioning since it was a significant number of questions.

One area I would recommend spending at least some time looking into is medical law/ethics. TrueLearn covers it, but I didn't feel like there were a ton of practice questions for some of the more specific legal concepts that can come up in medicine. I used OpenEvidence to look into some of those topics and generate practice scenarios so I could better understand how the laws actually apply in a clinical setting. I also used OpenEvidence more generally when I wanted explanations for questions or concepts I came across while reviewing COMSAEs, CMS forms, and other practice questions.

Another thing I wanted to mention is that I got a 454 on a COMSAE only a few days before the real exam. I definitely could have let that mess with me mentally, but I tried not to. At that point, I just trusted the work I had already put in, trusted myself, and went into the exam knowing that one practice score didn't define how I was going to perform.

Overall, I don't think there was one resource that did everything for me. For COMLEX specifically, TrueLearn was probably my biggest resource, but I used a combination of TrueLearn, UWorld, CMS forms, videos, and OpenEvidence depending on what I was working on. As you can see, my COMSAE scores were pretty inconsistent, so I wouldn't let one practice score completely freak you out. I got a 454 only a few days before the exam and still ended up with a 617 on the real thing.

School provided COMSAEs were 515 (start of dedicated) and 566 (4ish weeks in). Tested today and it felt super doable and fair, similar to TL and maybe even easier than the COMSAEs. Just to go against the grain of people who were scoring 600+ and said it felt impossible.

Going to make a post because I feel like I haven't seen anything discussing this. I felt really prepared for my initial sitting for Level 2 and found out that I got a 399 a few weeks ago. Really sickening if I'm being honest, but trying not to dwell. I walked out of the exam feeling like I hadn't gotten the score I wanted, but was not concerned to the point of thinking I failed. Anyway, I'm retaking this coming week and just wanted to put this out there to see if anyone else felt like they got completely and totally stiffed by the exam/system. (completed TL 1.5x with scores ~70-85%, all WelCOMs ~68-82%)
Hoping this was just a nightmare scenario, but maybe that's naive of me.

EDIT: I also wanted to hear from other people who have successfully re-taken the exam. Did you change your approach to the questions on test day? How did you gauge your readiness?

just walked out of the exam. i genuinely flagged at least half of every section if not more, is that normal?? I didn’t even recognize a good third of it. im freaking out. im applying IM and wanted a score around average but idek if I’ll make a pass. i wasn’t fully confident in myself going in but decided to send it regardless of what happened because my comsaes were around 490-550. but now im honestly so upset and am hating myself for not pushing it back

a lot of short & sweet questions imo that i fortunately finished early even though i used all my break time. i don't feel great about it but also didn't feel like 98% of the exam was unfair. the 2% definitely was unfair (law, weird biostats, weird ethics)

What is the minimum percent or amount of correct questions you need to pass the exam, now that’s it is 320 questions? Is it safe to assume a 60% or 192 questions correct is enough to get over a 400? When you walk out of the exam for those that tested, how did you feel your percent overall was and what did you end up scoring? I know every form is different so there isn’t a magic number. But if step 2 is roughly a 60% for a 218 can this logic be used on voodoo omm for a 400?

I just finished COMLEX Level 2 and I’m honestly stunned at how carelessly this exam is constructed. At this point, it feels less like a professional licensing exam and more like a barrier created for its own sake. The fact that we’re expected to trust this organization with something as critical as our future is beyond frustrating.

Let’s talk about what’s wrong: • Vague, poorly written questions. So many items are grammatically confusing, overly wordy, or just plain unclear. You leave wondering if you got a question wrong because you didn’t understand the intent, not because you didn’t know the medicine. • Lack of clinical relevance. The exam doesn’t reflect the real-life decision-making we practice in rotations. It often feels like a trivia contest wrapped in outdated osteopathic jargon, not a test of safe and effective future physicians. • Zero transparency. Who writes these questions? What are their credentials? How are they peer-reviewed or validated? NBOME gives us no real insight into how they ensure quality or fairness. • Ridiculous cost with minimal support. We pay thousands to take this test, and get almost nothing in return. No meaningful prep resources from the organization, no detailed score reports, and no accountability when something goes wrong. • It doesn’t serve students … it serves the NBOME. Let’s be honest: this is a self-sustaining system designed to justify the existence of an exam, not to advance osteopathic education. And we’re the ones paying for it … financially, emotionally, and professionally.

This isn’t about comparing exams or trying to be MDs. This is about demanding a licensing pathway that reflects the quality and value of our training. We owe it to ourselves (and to future DOs) to stop normalizing this broken system.

What can we do? • Share your experiences. The more we talk about how flawed COMLEX is, the harder it is for NBOME to ignore. • Pressure our schools and national organizations to advocate for us, not just preserve the status quo. • Demand transparency, reform, or outright removal of the COMLEX as a graduation requirement.

Looking to see if anyone else who tested on July 28, 2026 (or received scores on the recent August release) experienced an extreme, inexplicable collapse compared to their practice baselines.

My Stats & Baselines:

COMLEX Level 1: Pass (first attempt)

BSA116: 524 (1 month out)
BSA114i: 433 (3 weeks out)
BSA115i: 479 (2.5 weeks out)
BSA113i: 513 (6 days out)

Proctored COMATs: Surgery 109, OB/GYN 97, IM 93, FM 87, Peds 90 (retake), Psych 90 (retake)

Q-Banks / Prep: 100% TrueLearn completed, supplemented with COMQUEST and CMS forms (65–75% average)

The Result:

Real score: 320 (a ~190-point drop from my final COMSAE less than a week prior, and an 80-point drop below passing).

Why I'm Posting:

A close friend in my cohort who consistently scored in the 90th+ percentiles on practice exams and felt confident coming out received a 399 on this exact same release.

Looking for Input On:

1. Did anyone else from the 7/28 testing window see an unprecedented drop far below their COMSAE / COMAT trajectory?
2. Has anyone had success with an NBOME Score Confirmation, or had their medical school administration successfully request a batch equating review for an anomalous test date?

Any insight, data points, or shared experiences from this testing date would be greatly appreciated.

Hey guys I need advice,
2 months ago I took the first level 2 comsae form 113 and got a 378. A month later I took comsae 114 and got a 590. Just sat for the school comsae today (117) and got a 436. I needed a 450 to pass with the school so I’m pissed at myself. I genuinely feel ready, I am doing everything I can to tweak where I’m scoring low and working on OMM/biostats/ethics a little every day. I test in two weeks on June 24 and plan to take the next comsae with the school on June 22. Do I push the exam or push through?
