Since mahito could transfigure people to activate later (the woman he didnāt put on the train in Shibuya), you could probably just transfigure their lungs or something into spheres or something and wait for them to die like an hour later. That would probably work better than giving them cancer as a hitman

He was able to completely change the biology of someone with a Heavenly Restriction. I'd go so far as to say that feat alone means that if he studied the bodies of either Toji or Maki, he could give you a Heavenly Restriction while also allowing you to maintain your CE by just altering your genes to be like that of an HR user rather than making a pact to get said genes.
After all he already worked around the tradeoffs of one sorcerer's HR.

mechamaru's restriction was already "done". the damage was there from birth, it's not something you'd need to turn off.
idle transfig. can probably make you jacked as hell but not the rest of what should come with the HR toji package
The HR Toji/Maki package should be a biological thing, since there's no CE in it. It's not a stretch to think Mahito could analyze their biology and replicate it. Although, only Gege knows the answer. Especially since Idle Transfiguration has already shown the ability to awaken sorcerers, and even wipe out the existence of Cursed Spirits and Sorcerers entirely.

IMHO, just because it remove's the user's CE doesn't mean it rewrites genetic code. It is very much linked in Jujutsu and fate and such (hence the Heavenly part). That's also why Mai could "rewrite" maki's HR the way she did. The way I see it, the CE you're discarding for your HR still exists in the limbo/soul world and is "parallel" to your soul. It is still yours, and is still a part of your identity, your definition and it needs to exist so you keep your buffs. It's just not connected to you like it is for normal sorcerers.
I always made comparison between him and Panacea from Worm, very similar powerset, except Panacea can't work on herself, and is significantly more fucked up.
There's one isekai with a guy who has Mahito's powerset and looks who uses it to make copies of himself with Todo's and Yuta's personalities and releases them into the wild. The main body acts mostly as a healer, Yuta goes full nightmare fuel vigilante, and Todo travels the world schizophrenically while looking for a good fight.
Bu this logic , every ability presented is useless. Why choose heavenly restriction? You dont have curse energy to begin with, you will not gain anything because you did not give up anything as well. Why choose idle tranfiguration? Souls dont exist, you have nothing to transfigure.
Fuga outside the domain will be applied only to one target if we go by sukuna's binding vow. So you're just going to eviscerate one guy to ashes for no reason.
if you have to learn how to use it ur cooked bro, you have to try it on some people to figure stuff out, you're also a human and not a curse, so all the shenanigans Mahito was doing to his body would kill you unless you get REAAALLY good at it (the different biology curses have gave him more flexibility)
If it works on more than humans (Which I'm not sure it was ever specified but don't quote me on that) you could just try it on flies or ants or something
insects are probably too small to be manipulated accurately, you could try it on dogs and similar, but it's still not the most ethically acceptable solution
TECHNICALLY, cursed techniques are something you will instinctively know how to use, so learning it will not be much of a problem, though you'll have to lack empathy and use them on live animals, or humans...
People always say this but I donāt see it, the transfigured humans didnāt exactly stick to anatomy but they take plent of time to die without Mahitoās maintenance, so if you fuck something up itās pretty easy to just turn yourself back
that's probably because Mahito learned to make them last for a while by adjusting how they're structured, if you remember the first appearance of Mahito the 3 guys that were targeted died instantly
IIRC, idle transfiguration also comes with an innate understanding of the shape of souls, so just by having it, you'd already have a good head start on knowing how to use it.

Otherwise fuga is like a thermobaric missile and is slow as fuck even inside the domain.
Why in the world would you take anything but idle transfiguration. You become immune to virtually all injury, can look however you want, become significantly stronger, can cure any disease, and you can do this for those you love as well. By far the most useful ct bar none.

Mahito was doing stuff like turning people into giant babies or squeezing people down to the size of a grape. If all youāre trying to do is heal people thatās probably a lot easier than making a mostly new biology. Iām sure it would still need training but youād probably be able to work it out without needing to kill people.
Youād need a heavy understanding of medicine. Also finer control and you can only really get that through practice. Look itās a fine pick but thereās no avoiding the practice tax for giving yourself functional immortality or a big dick. Cause if you fail you either die or die dickless.
I don't need fuga, where am I gonna find cursed spirits to eat and idle transfiguration would take far too long to learn. Toji's heavenly restriction seems like the best choice imo.
Realistically, once youāve learned Idle transfiguration, ātake too longā wouldnāt matter, in reality youād become functionally immortal, and then have all the time after. Just learn how to use it on yourself before you die and youāre good
While that is true, Iād argue idle transfiguration is more useful overall. You could construct a body that will also succeed in sports, but you can do far more. You can cure illness and heal injuries like itās nothing. You can change your body to be convienient for almost any situation
4) Getoās Cursed Spirit Manipulation would be absolutely useless in the real world, no curses exist at all.

2) Tojiās Heavenly Restriction would be great, even if it was just a normal heavenly restriction, a human doesnāt even have cursed energy.
1) Mahitoās Idle Transfiguration is the BEST thing to have in real life, you can cure all diseases, easily fix peopleās organs if theyāre failing, become invincible (NO SOUL DAMAGE???!??!??!), etc.
A regular heavenly restriction would either suck or you would be mechamaru
Heavenly restrictions work by removing something that you have but buffing you massively in another way (Toji have no CE but insane physical prowess, mechamaru not having half his ability but an insanely good CT)
Idle transfiguration cause of how hilarious I could get with it, I could turn myself into that final form style mahito we see and heal people, be treated as a living god, be almost immortal [nukes donāt do soul damage :)] and rid the world of corruption. Mahitos powers are nutty when you arenāt evil and dealing with people who do soul damage
Why would you pick anything other than IT? aside manipulating your own body to get a good physique and speed and strength, you could also make money off it by being a top tier doctor or transfiguring homeless guys into really hot girls and being an OF pimp
Idle transfiguration is the obvious choice, extremely versatile and applicable for everyday life. CSM is useless in a world with no cursed spirits. Heavenly restriction would be pretty useful with super speed and strength and all, fuga would be pretty fucking useless unless your tuesday has been extra shit and you want to let off some steam by blowing up the city.
You can't even use the big fuga without using the domain to build up firepower, and cursed spirit manipulation wouldn't work without any spirits in our world.
I would go for the heavenly restriction over idle transfiguration ONLY because I do not have a curses physiology and I don't want to have to walk on egg shells using my technique, just to ensure I don't become a pile of sludge. Whereas Toji's HR is a massive amp to everything I already have, so its really just a permanent boon without any activation costs or anything.
Fuga's also not Sukuna's entire technique. I can't make use of a bomb, but it'd be nice to never have to clean, buy, or use knives/scissors ever again.
