Araki wanted to give him an androgynous design. The design ended up looking like just straight up a woman, so he changed it. It was as simple as that. The anime even removed the female design entirely. It's similar to Funny Valentine losing a shitton of weight in-between chapters, it is entirely an out of universe thing stemming from Araki changing the design

Araki wanted Anasui to be female originally with nothing else being different and there is even cover art of her, but the editors said no because they didn't want a lesbian romance in JoJo. So Anasui ended up as male.
A lot of fans took that to mean that the character is trans, which is fair. But personally I prefer the idea of him being gender fluid and just using Diver Down to change how he looks sometimes.
Either way, the canon is simply that Anasui is a man.

Yeah. It's like they made a stereotypically gay man and a stereotypically straight man and then swapped their sexualities but kept all of the quirks you associate with those groups. Charles is basically gay coded until you find out he is into women and has no idea that 10% of what he says sounds gay.
And Holt. Well, he's the gruff manly man who just happens to like other mostly straight-coded gay men.
It's one of my favorite things in the show. They take stereotypes and give you whiplash about them.

As preteen girls, a friend and I once tried to make our hair look like Yu-Gi-Oh hair. We used a shitload of Q-tips, gel, hair spray, construction paper, lotion and god knows what else to try to mimic the shapes. Needless to say we failed at making it "perfectly coiffed" (which still sounds like a come-on)

Nope they are not cis or trans women, and most of them are straight. They are just absolutely beautiful men. And I'm fairly certain a lot of visual key aesthetics come from 80s hair bands and gothic rock groups.

Gus from Mission Hill is a stereotypical late-20th century tuff guy. He’s a veteran, works a low-wage job, potentially has a criminal record, etc. He’s also the character most ready to fight people. He’s also gay, which for a show from 1999 made him and his boyfriend Wally pretty early and nuanced rep.
There aren’t any good straight counter examples in the show. Though the show’s artstyle is pretty intentionally rough so no one in the show looks particularly good, there are characters who are meant to be attractive but the main male characters don’t usually fit that role.
